Living Will Signings
Living wills and related health care directive documents may be prepared by an attorney, provided by a healthcare organization, or completed as part of a person's estate planning. The document should be completed before the notary appointment, except for any signatures intended to be signed in the notary's presence.
Each person whose signature is being notarized must personally appear before the notary, provide acceptable identification, be able to communicate with the notary in English, and sign willingly without coercion.
Living Wills in Hospitals & Care Facilities
Living will and health care directive notarizations can often be completed at hospitals, assisted living communities, skilled nursing facilities, rehabilitation centers, hospice facilities, and other healthcare settings.
Before scheduling a facility appointment, please confirm that the signer is available to meet with the notary and that the facility permits access at the requested appointment time. The signer must be able to personally participate in the notarization and communicate directly with the notary.
