Yes. I provide mobile notary services directly to hospitals, urgent care centers, and healthcare facilities throughout East Contra Costa County. I travel on-site to patient rooms, nurse stations, or designated meeting areas for convenient notarization.

For any notarization, the patient must:

• Be alert and aware

• Be willing to sign

• Be able to communicate clearly
  

If a patient is unconscious, heavily sedated, or unable to demonstrate awareness, I am legally unable to proceed.

If the signer is not fully aware or cannot communicate clearly, I am legally unable to complete the notarization. This is required under California law to protect the signer.

The signer must present valid, government-issued identification such as a driver’s license, state ID, or passport. If there are concerns about ID availability, I recommend contacting me in advance to discuss options.

Yes. I understand that healthcare situations can be time-sensitive. I offer same-day and evening appointments whenever possible to support urgent patient needs.

• Patient consent and affidavit forms

Some documents, such as advance healthcare directives or certain estate planning forms, may require witnesses. Requirements vary by document, so I recommend reviewing your paperwork in advance or contacting me with any questions. With advance notice, I can coordinate witnesses for your appointment for an additional fee, or you may arrange your own.

In many cases, hospital staff may act as witnesses, depending on facility policy and document requirements. It’s best to confirm with staff before the appointment.

If your document requires witnesses, they must be impartial adults who are not named in the document and do not have a financial or beneficial interest in it. Witnesses must be present at the time of signing and able to observe the signer.

In healthcare settings, it’s also important that witnesses are not directly involved in the patient’s care when possible, depending on the document requirements and facility policies.
