What Is an Advance Health Care Directive?
An Advance Health Care Directive is a document that allows a person to provide instructions regarding their health care and may allow them to designate another person to make health care decisions on their behalf.
Advance Health Care Directives can vary depending on how they have been prepared. If a signature requires notarization, the signer must personally appear before the notary and meet the requirements for the notarial act.
TRC Mobile Notary does not prepare Advance Health Care Directives, provide legal advice, or advise clients about the health care decisions contained in the document

Advance Health Care Directive Signings
Advance Health Care Directives may be prepared by an attorney, provided by a healthcare organization, or completed as part of a person's estate planning. The document should be completed before the notary appointment, except for any signatures that are intended to be signed in the notary's presence.
Each person whose signature is being notarized must personally appear before the notary, provide acceptable identification, be able to communicate with the notary in English, and sign willingly without coercion.
Hospital & Care Facility Signings
Advance Health Care Directive notarizations can often be completed at hospitals, assisted living communities, skilled nursing facilities, rehabilitation centers, and other healthcare settings.
Before scheduling a facility appointment, please confirm that the signer is available to meet with the notary and that the facility permits access at the requested appointment time. The signer must be able to personally participate in the notarization and communicate directly with the notary.
What to Have Ready
Before the appointment, please have:

• The completed, unsigned Advance Health Care Directive
• All required signers present
• Acceptable identification
• Any required witnesses
• Instructions from your attorney or document preparer
Want to make sure you're prepared?
Review our Mobile Notary Appointment Checklist before your appointment.
Important: The Notary's Role
TRC Mobile Notary does not prepare Advance Health Care Directives, provide legal or medical advice, interpret health care instructions, or determine which notarial act a signer should choose.
If an Advance Health Care Directive does not contain a notarial certificate or there is uncertainty about how it should be notarized, please contact the attorney, document preparer, healthcare organization, or other receiving organization for instructions before the appointment.
A notarization documents the notarial act performed. It does not determine the legal effectiveness of the Advance Health Care Directive or evaluate the health care decisions contained within it.
