Affidavit Signings
Affidavits may be prepared by an attorney, document preparer, business, government agency, financial institution, or other requesting organization. The document should be completed according to the instructions provided before the notary appointment.
Each person whose signature is being notarized must personally appear before the notary, provide acceptable identification, be able to communicate with the notary in English, and complete the requested notarial act willingly and without coercion.

With a jurat, the signer signs in the notary's presence and takes an oath or affirmation regarding the truthfulness of the statement. An acknowledgment involves the signer acknowledging that they executed the document.
A California notary cannot choose the notarial act for the signer. If the document does not contain a notarial certificate and you are unsure which notarial act is required, contact the attorney, document preparer, receiving organization, or other requesting party before your appointment.
What to Have Ready
